How do I obtain a copy of a death certificate for a death which occurred in Evanston?

The City of Evanston no longer has a Vitals Records office.  For deaths that occured in Cook County (including Evanston, IL), the death certficate may be obtained at any Cook County Clerk's office.  Please call 312-603-7790 or visit their website: https://cookcountyclerk.com/agency/birth-death-records
